Your After Visit Summary
Using click counts to give partial doses from a Zepbound KwikPen is an off-label method outside the manufacturer’s instructions. This guide is not approved or endorsed by the manufacturer. Lilly instructs users not to count clicks when selecting a dose.
The chart uses estimates and does not establish accurate partial-dose delivery. A disclaimer or patient agreement does not validate the method. Confirm your administration plan with your care team before injecting; do not use this guide alone to select an injection setting.
Your Zepbound plan
Pen disposal: Follow the manufacturer’s instructions to discard the pen after four doses or 30 days after first use, whichever comes first, even if medicine remains. Smaller doses do not extend these limits.
How to reduce nausea and vomiting
- Eat smaller portions: several small meals may be easier than one large meal. Use a smaller plate or bowl.
- Eat slowly: take small bites and pause. Stop when comfortably full; you do not have to finish your plate.
- Choose less greasy food: avoid large fried meals, heavy sauces, and lots of oil or butter. Try grilled or baked foods.
- Try simple foods when nauseated: small amounts of toast, crackers, or rice. Return to balanced meals as you improve.
- Sip fluids often: small sips may be easier than drinking a large amount at once. Inability to keep fluids down needs prompt medical attention.
- Include protein as tolerated: chicken, fish, eggs, beans, or tofu help support your muscles. Do not force a large meal when nauseated.
- Plan a consistent weekly injection day that fits your routine. Discuss schedule or dose changes with your care team.
- Contact your physician before increasing the dose if nausea or vomiting continues.
- If Zofran (ondansetron) is prescribed: use only as directed. It may help briefly with early nausea. Do not use it to cover up severe pain or persistent vomiting.
Some people have no side effects. Others notice nausea during the first day or two after an injection or after a dose increase. Persistent or severe symptoms should not be ignored.
Other possible side effects
Diarrhea, constipation, stomach discomfort, heartburn, burping, tiredness, injection-site irritation, and hair loss can occur. Contact your physician if symptoms continue or interfere with eating or drinking.
When to get urgent help
- Strong stomach pain that does not go away, especially going to your back: stop Zepbound and contact your physician immediately.
- Face, lip, tongue, or throat swelling; trouble breathing or swallowing; severe rash; or feeling faint: stop Zepbound and get immediate medical help.
- Cannot keep fluids down, very little urine, or marked dizziness: get urgent medical attention.
- Upper stomach pain with fever, yellow skin or eyes, or pale stools: contact your physician immediately.
- Shaking, sweating, dizziness, or confusion: blood sugar may be low, especially with insulin or certain diabetes medicines. Follow your low-blood-sugar treatment plan and contact your physician.
If you cannot be seen urgently in a timely fashion, treat this as an emergency and call 911.
Call 911 immediately for severe breathing difficulty, tongue or throat swelling affecting breathing, collapse, seizure, or unconsciousness. Do not wait for an appointment or give food or drink to someone who cannot swallow safely.
Keep your care team informed
Contact your physician promptly for ongoing vomiting or diarrhea, severe constipation, vision changes, a new neck lump, or persistent hoarseness.
Choose one realistic health goal alongside medication. Progress includes energy, strength, habits, and lab results · not just weight.
Tell your procedure team you take Zepbound before anesthesia or deep sedation.

Where to fill, what to pay, and when to refill
These routes use Lilly’s eligibility rules. TrumpRx does not remove those rules. The summary below covers the main cash-pay conditions, with full controlling terms linked for each program.
| Route | Format / fulfillment | Starting cash price | 45-day requirement |
|---|---|---|---|
| LillyDirect | Vials or four-dose KwikPen. Home delivery or Walmart pickup through LillyDirect. | $299 at 2.5 mg | Yes, to retain the $449 offer at 7.5–15 mg. |
| Local / retail pharmacy | KwikPen with the appropriate activated Lilly savings card. Confirm stock and card processing. | As low as $299 at 2.5 mg | Same higher-dose offer condition. Missing it affects the price, not a stated universal card-deactivation rule. |
| TrumpRx | Vial and KwikPen listings lead to LillyDirect. The KwikPen route also allows a retail transfer. | Manufacturer offer, by strength | Lilly’s underlying offer rules still apply. No separate refill exemption. |
The 45-day refill rule and prices
Complete the refill purchase within 45 days of the previous delivery/receipt date. Sending a prescription or requesting a refill is not the same as completing the purchase. Save the delivery date and confirm the deadline with the pharmacy.
- The $449 offer applies to 7.5, 10, 12.5, and 15 mg vials or KwikPens. Eligible new and existing patients receive it on their first qualifying purchase.
- For later purchases, the 45-day condition applies. If missed, the offer does not apply to that purchase. Do not assume a pharmacy transfer, dose change, or new card resets eligibility. Ask Lilly about the next qualifying fill.
- The 2.5 mg and 5 mg regular prices are $299 and $399. The higher-dose 45-day discount is not a condition of those regular prices.
- A program month is 28 days: four single-dose vials or one KwikPen containing four labeled doses. The 45-day window is a pricing condition, not an injection schedule or permission to use an opened pen longer.
| Strength per labeled dose | Qualifying 28-day price | Regular price without offer |
|---|---|---|
| 2.5 mg | $299 | $299 |
| 5 mg | $399 | $399 |
| 7.5 mg | $449 | $499 |
| 10, 12.5, or 15 mg | $449 | $699 |
Lilly may change or end the offer. Taxes, fees, and injection supplies may add to the price. Full Journey purchase terms.
How to obtain it through each route
- LillyDirect: Have the prescriber select the exact device and strength and send the prescription to LillyDirect Pharmacy. Complete its eligibility and payment steps, then choose available delivery or Walmart pickup. LillyDirect applies available self-pay savings automatically. Vials are exclusive to this self-pay channel, including its Walmart pickup option.
- Local pharmacy: For the KwikPen, send or transfer the prescription to the chosen pharmacy, enroll in the appropriate Lilly card, activate it, and provide the processing details. Confirm the final price before paying. Vial pickup through LillyDirect is different from filling a vial prescription at any independent retailer.
- TrumpRx: Choose the Zepbound vial or KwikPen listing, then follow the LillyDirect link. Lilly confirms eligibility and fulfillment. The official listings do not promise a lower price or a way around refill conditions. Compare any insured copay before choosing cash pay.
LillyDirect refill support: 844-559-3471. Availability depends on the product and pharmacy. Confirm current prescription-routing details with LillyDirect.
Retail KwikPen self-pay card: eligibility, limits, and exclusions
The KwikPen Self-Pay Savings Card is distinct from Lilly’s commercial-insurance card. Its terms require an approved-label prescription, age 18+, U.S./Puerto Rico residence, and cash payment. Insurance is not required. A commercial card has separate insurance requirements and excludes government-program beneficiaries. Select the correct program.
- Cash means outside insurance: No insurance or third-party reimbursement, including reimbursement accounts covered by the terms. Purchases do not count toward insurance deductibles or true out-of-pocket requirements.
- Card limits: Up to 11 fills per calendar year, each for a 28-day supply of one eligible KwikPen. Card savings end December 31, 2026. These are card-specific limits, not a blanket statement about every LillyDirect purchase.
- Maximum savings per fill: $215 at 2.5 mg, $115 at 5 mg, $65 at 7.5 mg, and $271 at 10–15 mg. These caps can affect the final retail price. Pay applicable taxes, fees, or charges beyond the benefit.
- Restrictions: Activate the card, meet eligibility every fill, and use only eligible KwikPen products. No stacking with other Zepbound discounts, transfers, resale, or counterfeit cards. Alternate-funding programs that require use of this assistance can make the patient ineligible. Lilly may change or end benefits, request a replacement card, or revoke use inconsistent with the terms.
The self-pay card does not cover single-dose auto-injector pens or vials. Complete self-pay card terms and current eligibility. These manufacturer offers require an approved-label prescription and do not establish eligibility for click-method use.
If the pharmacy charged more despite a timely qualifying refill
Lilly’s KwikPen card terms allow a post-transaction reimbursement request when the refill met the 45-day rule and other requirements but Lilly could not verify the timing at checkout. This does not waive a missed deadline or guarantee reimbursement.
Keep the original activated card details and pharmacy receipt. The claim requires the patient’s name, date of birth, address, and supporting documents. Submit through the official portal or the mail process in the terms by March 31, 2027. Lilly may request more evidence. Compare branded weight-management and diabetes medicines, including oral options. Prices are dated manufacturer self-pay references, not live pharmacy quotes. Offer eligibility matters. A prescription for the product’s approved use and other conditions may be required. “As little as $25” commercial copays are not cash prices. Self-pay purchases generally cannot be billed back to insurance. Confirm current terms with the dispensing pharmacy before purchase. These prices assume labeled products and quantities; they do not use click-method or partial-dose calculations.

Unofficial, unvalidated method
The Zepbound click-method chart displays estimates from a third-party source. This tool has not been clinically validated to establish the accuracy, safety, or effectiveness of partial-dose delivery. Click counting is outside the manufacturer’s instructions, which direct users not to count clicks when selecting a dose. This website is not affiliated with, sponsored by, or endorsed by Eli Lilly and Company.
Do not rely on this tool to select an injection setting
Outputs do not establish a prescription or confirm that a particular dose will be delivered. Patients should not use this website to start, stop, split, or change medication doses. Consult the prescribing clinician and pharmacist about medication administration.
Clinical risks and device limitations
Arithmetic does not validate partial-dose delivery or establish a longer usable supply.
- Discard limits and infection prevention: Discard the KwikPen after four weekly doses or 30 days after first use, whichever comes first, even if medicine remains. Other labeled storage limits still apply. Lilly’s testing supports the labeled in-use period, not extending one opened pen over two or three months. Use a new needle for every injection, remove it for storage, and never share the pen, even with a changed needle. Needle reuse or sharing can expose patients to infection.
- Uncertain dosing: Lilly does not recommend splitting KwikPen doses because this has not been studied. Click counts do not establish accurate partial-dose delivery. Miscounting or using the wrong pen strength can result in an unintended amount. Excess medication can cause harm, including gastrointestinal symptoms, and inadequate dosing may reduce treatment benefit. Do not independently repeat an injection to make up an uncertain amount.
- Device limits: The KwikPen is designed for four fixed doses. It prevents dialing a full dose after four weekly doses. Remaining liquid is not proof that another complete dose can be delivered. Do not override the device’s stops, attempt to extract leftovers, or transfer medicine into a syringe. If the pen cannot dial a full dose, follow Lilly’s instructions to discard it and contact the care team or Lilly for help.
For suspected overdose, seek immediate advice from the care team or poison control. Call 911 for severe breathing difficulty, collapse, seizure, or unconsciousness.
